o Purchase order number
o Recipient First and Last name
o Shipping address
o Billing address
o Phone number
o Email
o Product name, catalogue number and size (A or B or C or D or Custom order)
o Product Price (for quotation please provide Quotation number or special offer code)
o Shipping and handling (For your convenience the shipping and handling charges will be added to the order prior to shipment)
